Vincent Atinga opened the scoring from the penalty spot in the 13th minute after Ernest Nuamah won a spot-kick following a defensive error.
Abdul Aziz Issah doubled Ghana’s advantage four minutes after the restart, reacting quickest to a loose ball following a cross to fire home in the 49th minute.
But The Gambia pulled one back in the 82nd minute, with Mohamed Badamosi converting a penalty after a foul in the box.
The momentum shifted quickly as substitute Ebrima Ceesay levelled the score with a composed finish inside the area.
The Black Stars then struggled to contain the visitors, who capitalised on a goalkeeping error and a counter-attack to score twice in the closing stages and seal a 4-2 victory.
The defeat leaves Ghana with questions to answer after conceding four goals despite holding a two-goal advantage with less than 10 minutes of normal time remaining.
